Form 1004 is the standard appraisal form for single-family residential properties, required by Fannie Mae and Freddie Mac. It is the most widely used residential appraisal form in the United States.

- Standard: $350-600 for single-family homes
- FHA: $1-75 additional for FHA requirements
- Rural/complex: Higher fees for travel and analysis
- Paid by: Borrower at application or closing
